Family Connects Durham is a nonprofit offering universal early postpartum nurse visits to families in the comfort of their own home at no cost. If you are a Durham County resident, and are bringing home a newborn who is between 0-12 weeks old (often called the "4th trimester"), you are eligible to receive a visit from us! Our nurses focus on maternal and infant health, and can provide both basic health screenings for caregiver and newborn (blood pressure check for caregiver, weight check for newborn), brief lactation counseling, postpartum anxiety/depression screenings and referrals, as well as connections with other resources in the community such as parenting groups, car seat safety classes, food access, etc. Families typically receive between 1-3 visits.

Our community alignment specialist Derek will be presenting the program and available to answer questions or help you get connected with one of our program support specialists in order to enroll in the program. We can sign families up for a visit either in the third trimester (prenatally), or after birth. We hope to see you soon!
